In this post, I'm going to talk about accessories. Adding accessories to an outfit is a good way to make it more over-the-top, so it's definitely an important part of over-the-top gothic lolita.
Hats: Various kinds of hats make for excellent crowning touches to an over-the-top gothic lolita outfit. Some appropriately aristocratic and darkly romantic would be:
Bonnets.
Top hats (including adorable tiny ones).
Decorated tricorn hats (I always think of them as "Lady Maria Hats", because they look like the hat that Lady Maria from Bloodborne wears).
Other gothic hats.
Hair decorations: Hats aren't the only types of head accessories. Two other types of hair decorations are headbands and hair corsages. Both headbands and hair corsages have a distinct romantic charm. To make the outfit even more over-the-top, hair decorations can be worn in combination. A headband can also be worn with a hair corsage, and a hair corsage can be worn with another hair corsage.

Arm covers: Arm covers are essentially fingerlesss gloves. They are a way of decorating the arms, and also keep the arms warm (often needed, because lolita dresses usually have short sleeves). Arm covers can be of any length, from covering only the wrist to going over the elbow. While arm covers are already a way of making an outfit more elaborate by decorating the arms, they can be made even fancier by having the arm covers be decorated with things such as ribbons, lace, charms, jewels, or even feathers.

Collars: Collars are one of the most quintessentially gothic accessories. A collar is an easy way to make an outfit more darkly elegant and romantic. There all sorts of decorations that can be used to make collars fancier, such as crystals, lace, charms, ribbons, fabric flowers, metal charms, fancy fabrics, feathers, and centerpieces like large jewels, cameos, or even (in one dramatic example I found) artificial bird's-skulls.

Sunglasses: Wearing sunglasses with a gothic design is an easy way of adding an air of mystery to an outfit.

To go in an edgier but still darkly romantic direction, you can wear a collar made of leather, preferably with the leather cut into gothic designs.
For added drama, use a collar that extends over the shoulders or chest. This creates a more extravagant style, and also decorates more of the body.
