No, I did not forget about this! I just spent a lot of time accumulating images as illustrations.
One way to increase the elaborateness of a gothic lolita outfit is with fancy accessories. Gothic lolita accessories tend to be fairly simple (at least when compared with those in sweet and classic lolita), so fancy accessories are an easy way to make an outfit more over-the-top.
For example, the most common shoes in gothic lolita are fairly plain Mary Jane shoes. Some alternatives that would be more over-the-top would be:
Fancy heels.
"Witchy" shoes.
Victorian-style boots.
One type of shoe that would be excellent for OTT gothic lolita would be leather boots decorated with buckles, straps, ribbons, faux crystals or pearls, or fancy stitching. These shoes are fairly common in western goth, but I've never actually seen them with gothic lolita. Which is unfortunate, because I think they would be perfect. I also think plain but elegant black leather riding boots could work very well, also. Not as fancy, but they have a romantic darkness that is perfect for gothic fashion.
Another easy addition to an outfit to make it more over-the-top are gloves. Lace, leather, and satin gloves all indicate dramatic elegance.
Bags with gothic-motif decorations or shapes also make an outfit more over the top.
Bags can be further decorated by tying ribbons, lace, necklaces or strings of beads (think rosarys), or charms (preferably with a gothic theme) to their straps. One thing I've occasionally seen (and have included some examples of) that would be very appropriate for OTT gothic lolita would be bags designed to look like books, specifically, gothic novels or collections of gothic short stories. Alternatively, it would be cool to see bags designed to look like occult tomes or alchemy books or even a witch hunter's Bible.
One final type of accessory that adds both "over-the-topness" and "gothicness" (for lack of better words)? Capes.
For extra-extra fanciness, consider a cape with fur or beading on it.
